What would make the steering wheel on my 2002 Ford Focus shudder?

When I'm driving along the motorway the steering wheel shudders now and again. It seems to happen after I brake,it goes on for a minute or two then fades away. I swapped my front/back wheels and had them balanced, seemed to help for a while but its back

You've had a lot of elaborate answers (most of which are right), it is the front brake discs (rotors only if you are American) warped, pushing the pads forward and back in the callipers and therefore pulsing back the brake pedal at you.
